California facility licensing and enrollment guide
ARF & RCFE Licensing. ALW Enrollment. Level 6 Vendorization.
Understand your options, compare RCCS consulting fees, estimate agency payments, and see the steps from a new-facility application to program enrollment. A transparent, interactive and nonbinding resource for California care operators.
Important: This guide is informational and nonbinding. No payment or contract is collected on this page. RCCS fees and agency payments are separate, and only the responsible government body can approve licensing or enrollment.
01 / One simple project planner
Choose your project first. Review your estimate afterward.
Complete the facility information and service choices in order. Your personalized costs and government review estimates appear below all selections, so there is no need to scroll back and forth. Fees paid to RCCS and fees paid directly to agencies are always separated.
New to residential care? Here's the short version.
1. Choose the facility. ARF is usually adult care; RCFE is usually senior care.
2. Get the license. CDSS, the state licensing agency, must approve the facility.
3. Choose an optional program. ALW or IRC Level 6 has separate enrollment after licensing.
STEP 1 OF 4
Choose your facility type
ARF generally serves adults who need supervision; RCFE generally serves older adults. Select the type that fits your planned care model.
The RCCS licensing package includes application preparation, a mock pre-licensing inspection, corrections to the documents covered by your package, and follow-up with CDSS through the application decision. Optional RCCS fire pre-inspection is $500 extra. Government filing fees are paid by you directly to the agency.
STEP 2 OF 4
Tell us about your facility
These details determine which government fees and approval steps appear in your estimate.
STEP 3 OF 4
Choose RCCS services
Choose only the services you want to include in the estimate. If you select no service, the total and timeline will remain unselected.
For this RCFE estimate, ALW is the only optional follow-on program.
Year selection: 2026 and 2027 RCCS package prices are shown as provided. CDSS/DHCS amounts are current reference fees, subject to agency updates. A future fire inspection price has not been confirmed.
STEP 4 OF 4
Your fees and projected review timeline
This estimate reflects only the selections above. Government processing begins when the correct application is filed and cannot be guaranteed.
Optional ALW enrollment is available in this RCCS calculator.

Your selected services and costs
No service selected yet. Choose a package to begin.
Select licensing or an optional program to see the relevant government review window.
A new facility must become CDSS-licensed before its program submission. CDSS licensing is not included in this estimate unless you separately select it above.
Combined listed amount (no services selected)
This is the sum of selected RCCS professional fees and listed agency charges—not a complete facility startup budget, invoice or guaranteed quote.
Excluded or not yet priced: Property, staffing, insurance, permits, fingerprinting and other startup costs are excluded.
| RCCS service | Through 2026 | Starting 2027 | Core inclusions |
|---|---|---|---|
| ARF / RCFE license application | $3,500 | $4,500 | Application packet, pre-licensing inspection, corrections and follow-through |
| ALW enrollment package | $5,000 | $6,000 | DHCS ALW program and separate Medi-Cal enrollment documents |
| IRC Level 6 ARF vendorization | $5,000 | $6,000 | Qualifying IRC program design and vendorization preparation |
| Fire pre-inspection (optional) | $500 | Confirm with RCCS | Additional consultant readiness inspection; not official fire clearance |
Government payments are not RCCS revenue. You write checks or obtain cashier’s checks payable to the government entities specified in their own instructions. RCCS may package your separately written CDSS check for filing. No RCCS service fee includes an agency filing fee unless expressly described in a signed agreement.
Want help preparing your application?
Review the RCCS licensing package or book a focused 30-minute consultation before committing to a project.
02 / From signature forms to follow-through
What happens after you purchase licensing support?
RCCS sends your signing forms within two business days after purchase. Before we mail your license application to the state, the ownership, finances, administrator, facility documents, and applicable property requirements must already be ready. The state controls the approval date.
Purchase & signatures
Within two business days after purchase, RCCS emails the forms for you to print, sign and return by mail.
Assemble & share
After all required prerequisites and signed originals arrive, we assemble the application, then send you scanned copies before mailing.
File & follow up
We mail your application and the agency check you prepared to CDSS. We follow up and copy you on our follow-up emails.
Agency determination
CDSS reviews and decides on the license. If you also purchased an optional program, its formal enrollment can proceed after the facility license is active.
What RCCS continues to do
- Unlimited corrections within the agreed licensing scope.
- Ongoing CDSS application status follow-up.
- Applicant copied on RCCS follow-up emails.
- RCCS listed as an authorized correction contact where accepted by CDSS.
- Assistance through the agency decision on the originally contracted application.
Required before mailing the CDSS packet
Both preparation groups must be completed before submission—not after mailing.
- Ownership and applicant readiness: ownership and entity information, acceptable proof of funds, administrator readiness and required orientation completed.
- Facility and documentation readiness: floor plan, required policies, fire-readiness preparation and applicable local site requirements completed or verified for submission.
Different stage: After the CDSS packet is mailed, an RCFE may prepare its ALW nursing/NPI records, or an ARF may develop its IRC Level 6 program and staffing model while awaiting licensure. Formal follow-on submissions come after an active CDSS license under the RCCS process.
| Milestone | Estimated timing | Key condition |
|---|
Important: CDSS licensing and program enrollment remain government decisions. RCCS cannot promise faster approval, access to waitlists or special priority.
03 / Deliverables and service commitment
What each consulting package includes
The services below describe RCCS's standard scope for a qualifying project. Outside fees, substantial property changes and additional license applications are separate.
ARF / RCFE licensing
- New-facility licensing application preparation.
- Applicable plan of operation, policies and supporting records.
- RCCS pre-licensing inspection included.
- Signature document email within 2 business days after purchase.
- PDF packet shared before mailing to Sacramento.
- Unlimited in-scope CDSS corrections and agency follow-up.
- RCCS-error refund protection under qualifying conditions.
ALW enrollment
- Eligibility and current license-readiness checklist.
- DHCS ALW program application for PFSRU.
- Separate Medi-Cal enrollment package for PEU.
- Wet signatures, notary, NPI, entity and insurance coordination guidance.
- On-call or employed RN documentation planning.
- In-scope correction and verification support.
IRC Level 6 ARF vendorization
- Facility model and readiness assessment.
- Program design and level-of-service documentation guidance.
- Vendorization form and supporting record preparation.
- IRC corrections and communication within the agreement scope.
- High-needs staffing, site and operational planning checklist.
- Rates, program approval and referrals remain IRC decisions.
RCCS-error money-back guarantee
The RCCS professional licensing-package fee may be refunded when an error solely caused by RCCS:
- causes rejection or denial without a viable correction pathway; or
- causes a significant delay exceeding 90 days.
Excludes unrelated CDSS delays, property/fire deficiencies, missing or inaccurate applicant information, material scope changes, non-RCCS advice, and government or third-party charges. The signed agreement controls specific eligibility and remedies. This is not a guarantee of approval.
Communication and turnaround
RCCS emails signature forms within two business days after purchase. You sign and mail required originals. Before submission, the required ownership, financial, administrator, orientation, floor-plan, policy, fire-readiness and applicable local-site items must be complete. We share a scanned PDF of the assembled packet before submitting it to CDSS.
We continue following up with CDSS and copy the client on our correspondence, requesting authorized consultant correspondence where permitted.
Full packet turnaround depends on the client's document completeness and property readiness; this is separate from the two-business-day forms commitment.
Usually excluded unless expressly purchased or agreed: agency fees and checks, local permits, official fire inspections, construction or engineering, administrator certification, insurance and bond purchases, notarization, fingerprinting, RN employment, staffing, payroll, facility management, medical billing, legal representation and resident placements. Optional RCCS fire pre-inspection is $500 currently.
04 / New versus existing facility
New facility vs. already licensed facility
A current license can avoid an entirely new CDSS filing in a suitable follow-on enrollment project, but it does not automatically establish program eligibility.
| Topic | New / unlicensed facility | Already licensed facility |
|---|---|---|
| CDSS | New application packet, official review and licensure. | Verify active license, good standing, and any required license modification. |
| Operations | Prepare the site, administrator, staffing, fire readiness and documentation before approval. | Maintain compliance, records, staff training and safe operations. |
| ALW | Prepare materials early; formal RCCS submission follows an active CDSS license. | May proceed to separate DHCS program/Medi-Cal enrollment if eligible. |
| IRC Level 6 | Program design readiness can begin early; formal vendorization is a later step for eligible ARFs. | Qualifying active ARF may pursue separate IRC vendorization. |
| Approval timing | Licensing approval is a prerequisite to the selected agency follow-on path. | Licensing wait may be bypassed; program reviews remain separate. |
| Fees | Licensing, startup, and separately selected program/application costs. | Selected ALW/IRC professional and agency costs; other compliance expenses may arise. |
05 / DHCS assisted living waiver
ALW requires two separate application paths
The Assisted Living Waiver is distinct from CDSS licensing. For a new facility, RCCS first requires an approved, active CDSS facility number before formal ALW submissions. An eligible already licensed facility may proceed to DHCS review.
01 / Documents
Preparation and readiness
About 2–4 weeks for packet preparation after all required materials are received, as a planning target.
- Active CDSS license and good standing
- NPI, entity, tax, insurance and facility records
- On-call or employed licensed nursing arrangements
- Staffing, operations and physical-plant readiness
02 / Applications
DHCS PFSRU + PEU
Program application: sent to DHCS PFSRU according to current submission instructions.
Medi-Cal enrollment: separate PEU packet with wet signatures, notarization where required and a $730 cashier’s check paid directly to DHCS.
03 / Agency approval
Reviews and verification
DHCS can request corrections and conduct on- or off-site verification.
Approximately 6–12 months or longer is an RCCS planning estimate, not an official guarantee or expedited processing promise.
Not the same waitlist: Facility ALW provider enrollment is distinct from the participant waitlist for eligible Medi-Cal beneficiaries. Care Coordination Agencies, resident suitability, authorization, available program capacity and admission decisions remain separate.
06 / Our operational experience
Experience beyond preparing paperwork
Ontario Level 6 ARF operations
RCCS leadership reports operational and management responsibility for three Level 6 ARFs in Ontario serving individuals through Inland Regional Center.
Our team's working relationships with Inland Regional Center Consumer Services Coordinators support practical familiarity with care programs, documentation and operating expectations. They are not agency endorsements or a promise of faster vendorization.
Affiliated RCFE referral coordination
Through affiliated Rosenthal Adult & Senior Care Connections (RASCC), qualifying RCFEs may have access to assistance identifying potential resident inquiries through local care networks, including cases involving IEHP, Kaiser and CalAIM-related arrangements when applicable.
Referrals depend on eligibility, authorizations, preferences and appropriate placement. No volume or payer approval is guaranteed.
Ready for practical support?
RCCS helps prepare and follow through on the work described above. You do not need to submit a long quotation form.
07 / Questions from prospective operators
Frequently asked questions
Pick only the questions relevant to you. Definitions of unfamiliar terms are below; each answer expands when selected.
New owner? Plain-English guide to common licensing terms
California Department of Social Services—the state agency that licenses ARF and RCFE facilities.
Approval to provide qualifying services through a Regional Center program. It is separate from CDSS licensing.
Assisted Living Waiver—a California Medi-Cal program with separate enrollment requirements.
Inland Regional Center, which coordinates services for eligible people with developmental disabilities in its service area.
Financial documents showing that you have the required resources to start and operate the facility.
Edits to application documents covered by your RCCS service agreement, not unlimited new projects or unrelated work.
National Provider Identifier—an identification number used for certain healthcare and enrollment processes.
Department of Health Care Services—the California agency overseeing Medi-Cal and ALW enrollment.
Do I pay CDSS, DHCS, or IRC agency fees to RCCS?
No. Agency fees are separate from RCCS consulting fees. Applicants write the applicable check or cashier’s check directly to the responsible agency, following its instructions. RCCS can include a client-prepared CDSS check with the packet. The DHCS Medi-Cal application currently lists a $730 cashier’s check, and CDSS online orientation is paid directly through the registration system if needed.
Why do costs and timelines change between ARF and RCFE?
ARF and RCFE have different CDSS application fees. In this RCCS cost explorer, RCFE can select ALW enrollment and ARF can select IRC Level 6 vendorization; the opposite program cannot be selected. This represents the consulting packages configured here, not a blanket statement of government program eligibility. New facilities require active CDSS licensure before the selected follow-on application is formally submitted.
What does the $3,500 ARF/RCFE licensing package include?
The standard new-facility package includes application documents, applicable supporting policies, RCCS pre-licensing inspection, unlimited in-scope corrections, a PDF packet copy before mailing and ongoing CDSS application follow-up. The stated package rate increases to $4,500 beginning 2027. Optional consultant fire pre-inspection is separately priced at $500 currently.
When will I receive documents to sign?
No later than two business days after purchasing the licensing package, RCCS emails the applicant the forms to print, sign and mail back. RCCS mails the completed packet only after receiving the required originals, agency payment instrument and completed pre-submission items: ownership, proof of funds, administrator readiness/orientation, floor plan, policies, fire readiness and applicable local site requirements.
Will RCCS continue helping after the application is mailed?
Yes. RCCS follows up on the original CDSS application and handles unlimited document-related corrections within the contracted scope. RCCS copies the client on its follow-up emails and asks CDSS to include RCCS in correspondence. The state may also communicate directly with the applicant.
Can approval be guaranteed in two to four months?
No. Approximately 2–4 months is an estimated CDSS processing window used for planning. Timelines can be longer because of inspections, corrections, fire clearance, applicant readiness, backlog or other factors outside RCCS control.
Can ALW or IRC Level 6 vendorization begin before licensure?
Preparation such as nursing arrangements, program design, document collection and readiness work can begin earlier. Under RCCS’s proposed sequencing for a new facility, formal follow-on ALW or IRC vendorization submissions occur after the CDSS facility license number is active and approved.
Does ALW enrollment involve two applications?
Yes. A qualifying facility must complete a DHCS ALW program application and a separate Medi-Cal provider enrollment process, with its own documents, filing requirements and any published fee. These are not the same as obtaining the underlying CDSS facility license.
Does a facility need nursing coverage for ALW?
DHCS facility requirements include arrangements for an employed or on-call licensed nurse and other standards concerning staffing, services, physical environment and records. RCCS can help organize documentation; actual nursing services, staffing costs and site modifications are outside the application package unless separately agreed.
How long does ALW enrollment or Level 6 vendorization take?
RCCS uses preliminary planning ranges of around 6–12 months for RCFE ALW enrollment and 9–12 months for ARF IRC Level 6 vendorization, with longer timelines possible. These are not official agency guarantees. The calculator shows only the timeline for the selected facility type and follow-on service.
Are government-funded referrals guaranteed?
No. Affiliated Rosenthal Adult & Senior Care Connections may help coordinate suitable RCFE referral inquiries involving IEHP, Kaiser or CalAIM-related arrangements where appropriate. Neither facility licensing nor RCCS/IRC relationships guarantee placements, payer approval or reimbursement.
What triggers the licensing money-back guarantee?
The limited refund applies to the RCCS licensing professional fee when a mistake solely attributable to RCCS causes a denial with no viable correction path or causes a significant delay exceeding 90 days. Government fees, normal agency delays, omissions by clients, property deficiencies and other listed exclusions are not reimbursed. The executed agreement sets binding terms.
What if the project is already licensed?
An already licensed, eligible facility may be able to pursue the separately selected ALW or IRC Level 6 program without repeating a new CDSS application. Active status, good standing, ownership, facility type and program eligibility still must be checked.
